Meet the Team

Owner, Support Coordinator, ABISC, QIDP
Tracy Chapman
Tracy has been working with people with disabilities for over 20 years. She has been a Support Coordinator for ten years and is truly passionate about working with this population. She has experience in almost every aspect of care for people with disabilities.
​
Tracy serves clients throughout all of Utah and maintains excellent relationships with all of her clients and their families. She has connections with all of the top providers in Utah and is a go-to for many other Support Coordinators in the state for her knowledge and expertise. Her clients love that she shows up to their programs and performances and often takes them to do fun things for her monthly visits. She has strong connections with the DCFS community and has several clients who are also in DCFS. She is especially passionate about working with kids.

Tracy is a mom of four, a step-mom of four, and a grandma to 22 amazing children. Spending time with them is her favorite past-time. She also loves golfing, skiing, hiking, reading, traveling, and a good (or not so good) murder mystery.

Support Coordinator, RN, QIDP
Aubree Rasmussen
Aubree has over 12 years of experience working with people with disabilities in different capacities. Her passion is improving peoples' lives and she loves that she gets to do this for her job. She keeps a small caseload allowing her to get to know each client well and be available for them when they need her, day or night, weekday or weekend. She recognizes she is working with people not products and she strives to be available to them as much as possible. Aubree is committed to finding solutions for her clients and will not stop until they are happy and their needs are met.

Aubree is a mother of four children and also has a background in pediatric nursing, equipping her to advocate for her clients and making her especially capable of serving those who are medically fragile.
She enjoys all things sports and outdoors, especially tennis, hiking, mountain biking and skiing.

Support Coordinator, QIDP
Jenny Noel
Jenny has over 7 years of experience working in the social service industry in different capacities, most recently working as a direct caregiver with Giv.Care. She loves working with this population and strives to help her clients live their best life.
She keeps a small caseload allowing her to get to know each client well and be available for them when they need her, day or night, weekday or weekend. She thinks of her clients and their families as friends, not products. Jenny is committed to finding solutions for her clients making sure their needs are met.
Jenny is a mother of four children, including a set of twins. While her children were very young, she owned and managed a daycare out of her home. She enjoys working with all ages but especially loves working with children.
Jenny likes reading, listening to podcasts and spending time with friends and family.
